PA Bureau of Workers' Compensation is Phasing Out Paper Submission of Forms

Special Workers' Compensation Alert - Pennsylvania

On July 8, 2020, the PA Bureau of Workers’ Compensation sent out an email advising stakeholders that electronic submission of Bureau documents was being requested. The Bureau is moving toward the complete elimination of paper filing. The preferred method for submission of Bureau forms and documents is now via WCAIS upload. This process will allow the Bureau to expedite the processing of all forms.

The Bureau is specifically requesting that the following forms now be exclusively uploaded via WCAIS and no longer submitted on paper by mail:

  • Agreement for Compensation for Disability or Permanent Injury (LIBC-336)

  • Supplemental Agreement for Compensation for Disability or Permanent Injury (LIBC-337)
  • Agreement for Compensation for Death (LIBC-339)

  • Agreement to Stop Weekly Workers' Compensation Payments (LIBC-340)

  • Third Party Settlement Agreement (LIBC-380)

  • Notification of Suspension or Modification (LIBC-751)

  • Statement of Wages (both LIBC-494A and LIBC-494C)

  • Notice of Workers' Compensation Disability Status (LIBC-764)

  • Marriage or Death Certificates

These documents should be uploaded to WCAIS using the “Actions” tab by clicking on the “Upload Documents” link and selecting the appropriate document type from the dropdown list. If you cannot locate the form you wish to upload in the dropdown list, then you may upload any other forms in WCAIS 24 hours a day, seven days a week on the “Documents and Correspondence” tab using the “Upload Documents” link. Both tabs offer a free-form text box which allows you to enter a description of the document you are adding to the claim. Please remember that uploaded documents cannot exceed 10 MB in size.
